The intended use of this form is for you to give us the required preliminary information by which we can reserve your instrument, and prepare your rental agreement. A drivers license and credit card issued in the name of the renter are required for each rental. You can provide this information to us at the time your instrument is picked up. Please submit multiple instrument requests individually.

Important notice for Fall Rental Customers:
Because our rates are so inexpensive and because our existing customers are always referring their friends to us, we can occassionally can run short of specific instruments. You can be sure you get the best deal by reserving or renting online. Once your rental or reservation is confirmed there is no need to worry about getting your instument since it will be reserved for you.
